AFP Releases New Health Care Polling: Voters Demand Lower Health Care Costs and Greater Control Over Their Coverage

80% of voters support directing financial assistance to individuals, not insurers.

ARLINGTON, VA – A new national survey, conducted by Public Opinion Strategies on behalf of Americans for Prosperity (AFP), reveals a clear message from voters: health care costs are too high, government and insurance companies have too much control, and Americans want reforms that put them back at the center of the system.

A survey of 1,000 registered voters, conducted from December 5 to 10, 2025 (credibility interval ±3.53%), reveals overwhelming public support for policies that expand choice, flexibility, and personalized care.

“Voters are demanding relief from the high cost of health care, but only a small minority of them prefer government-driven ‘solutions.’ Americans overwhelmingly prefer personalized solutions like Health Savings Accounts and Direct Primary Care because they know these approaches give them real flexibility and transparency,” said Dean Clancy, Senior Health Policy Fellow at Americans for Prosperity. “Eight in ten voters support directing financial assistance to individuals, not insurers. Voters are looking for leaders who trust individuals to make decisions for themselves and who are willing to move power out of government hands and back to the people.”

Health Care Costs Dominate Voter Priorities

Reducing the cost of living (69%) and lowering health care costs (44%) top voters’ concerns by wide margins. An overwhelming 82% say bringing down health care costs should be a top priority in Congress, and 65% expect candidates to have a clear plan to do so.

Voters Reject Government-Driven Systems in Favor of Personalized Care

Voters report a major gap between who should have control over coverage decisions and who does:

77% say they and their families should have the most control, yet only 15% feel they do.
71% believe government or insurance companies currently hold the power, though only 19% say they should.

By an 83–17% margin, voters prefer reforms that increase personalized options rather than expand government control or subsidies—representing a dramatic 46-point net gain since 2022.

Strong Support for Consumer-Driven Solutions

Voters overwhelmingly back policies that give people the flexibility to choose the coverage and care options that work for them:

80% support directing financial assistance to individuals, not insurers.
78% approve of Direct Primary Care, and 75% want Congress to make DPC more available.
83% say Health Savings Accounts are a good tool for reducing costs, and more than 8 in 10 support making HSAs available to all Americans and usable to purchase any insurance plan they choose.

Given competing arguments for and against HSAs, 69% sided with supporters of HSAs, compared to just 31% who agreed with opponents.

“This polling confirms what American families have been feeling for years: the health care system is failing them,” said Lauren Stewart, Senior Legislative Affairs Liaison, at Americans for Prosperity. “People want lower costs, more choices, and the freedom to control their own care—not a system where Washington bureaucrats and big insurance companies call the shots. Lawmakers should take this as a clear mandate to put patients first.”

The key insight from the data shows that across the political spectrum, today’s health care system is viewed as too expensive, too complicated, and too controlled by government and insurance companies. People want meaningful reforms that reduce costs and give more control to individuals and families. Policymakers who support affordability, transparency, and consumer choice will gain broad and lasting public support.
